What Is DOT Drug Testing?
DOT drug testing is federally required drug and alcohol testing for employees in safety-sensitive jobs regulated by the U.S. Department of Transportation. It follows strict rules set out in 49 CFR Part 40 — covering everything from how specimens are collected to how results are reviewed and reported.
If you employ commercial drivers (CDL), pilots, railroad workers, transit employees, pipeline workers, or maritime workers, you likely need DOT-compliant drug testing.
Which Employers Need DOT Drug Testing?
Your employees need DOT drug testing if they work in safety-sensitive roles under any of these agencies:
- FMCSA — Commercial truck and bus drivers (CDL holders)
- FAA — Aviation workers (pilots, mechanics, air traffic control)
- FRA — Railroad employees
- FTA — Mass transit workers
- PHMSA — Pipeline operators
- USCG — Maritime workers

What Types of Tests Are Required?
Under DOT rules, employers must test covered employees in these situations:
- Pre-employment — Before starting a safety-sensitive role
- Random — Unannounced draws from your employee pool throughout the year
- Post-accident — After qualifying accidents or incidents
- Reasonable suspicion — When a trained supervisor documents signs of impairment
- Return-to-duty — After a violation, before returning to safety-sensitive work
- Follow-up — Ongoing testing after return-to-duty
What Is Tested in a DOT Drug Test?
The standard DOT drug panel tests for 5 drug categories:
- Marijuana (THC)
- Cocaine
- Opioids (including codeine, heroin, morphine, hydrocodone, oxycodone)
- Amphetamines (including methamphetamine and MDMA)
- Phencyclidine (PCP)
Most DOT drug tests use a urine specimen. Oral fluid is now authorized for certain modes — confirm with your DER and TPA before switching.
What Is an MRO (Medical Review Officer)?
An MRO is a licensed physician who reviews non-negative drug test results before they're reported to your company. The MRO may contact the employee to ask about legitimate prescriptions or medical explanations — then issues a final verified result. This protects both the employer and the employee from unfair outcomes.
